MDVA Grants to Counties program - Fiscal Year 2011
Thank you for your participation throughout the FY10 and FY11 Enhancement Grant cycles. At this time, many of you have either a mid grant report or end grant report due to the Minnesota Department of Veterans Affairs.
Please see the two categories listed below, FY10 Enhancement Grants and FY11 Enhancement Grants and select the proper report template from the list. Please make sure you select the appropriate template for the grant cycle (FY10 or FY11) and the grant category (transportation, outreach, etc.). For all End Grant Reports please include the Equipment Detail Form.
Please note that all reports from FY10 forward, must include a signature and date of the CVSO completing the report. The Minnesota Department of Veterans Affairs is following new reporting requirements set forth by the Office of Grants Management which requires that this information be included on all reports. You can scan, email, fax, or mail these back to Nicole Peine.
